i had the same problem, though mine was usually from discomfort. i also liked to prolong the pleasure and would do both by sucking until it was too uncomfortable, then masturbating by hand until i started approaching orgasm, then sucking some more until it got uncomfortable again, and so on.
when i was in better shape, i did manage to use the 'mouth only' technique to orgasm a few times, but there were also times i tried that and just couldn't seem to get close until i took things 'in hand' for a while before finishing up by mouth.
