It's from an American TV showed called Shameless. This scene appeared in season 3, but I'm not sure which episode. There's a movie called Short Bus that shows a guy autofellating with cum shot and all, and it was only rated R, not X. However, I couldn't find a link to it.

I saw Shortbus in the theater, that was an experience. I've been meaning to buy it on DVD but I guess I waited too long. The price went way up. Perhaps it's now out of print...
